Home Health Aide (HHA) Services
NYS-certified aides who combine daily personal support with basic health monitoring — every case directed and supervised by a registered nurse.
Care with a clinical backbone
A home health aide does everything a personal care aide does — and adds a layer of health awareness. HHAs complete a state-approved certification program that trains them to take vital signs, watch for changes in condition, and work from a plan of care written by a nurse.
That structure matters most after a hospital discharge, with chronic conditions like diabetes or heart disease, or whenever a doctor wants eyes on a patient between visits. Your aide reports to our nursing team; our nursing team keeps your family and physicians informed.
What an HHA helps with
- Vital signs — temperature, pulse, blood pressure
- Observing and reporting changes in condition
- Medication reminders on schedule
- Support with prescribed exercise routines
- Nutritious meal preparation, including special diets
- Escort to medical appointments
- All personal care tasks — bathing, dressing, transfers
- Documentation on every visit via EVV
A good fit when…
There's a health condition in the picture — a recent hospitalization, a chronic illness, medications that must stay on schedule — alongside everyday needs like bathing or meals. This is the most commonly authorized service under MLTC plans.
Consider more support when…
Your loved one can't be alone at night or between shifts. Live-in and 24-hour care keeps a certified aide in the home continuously.
